Dr Paul Baughan, a GP in Forth Valley and National Clinical Lead with Healthcare Improvement Scotland, has made this short video to explain the importance of care planning conversations for those most vulnerable to coronavirus infection.
Paul says: “I have been calling my highest risk patients to talk about the plans we should make, in case they become ill with COVID-19. A call with someone from your GP practice will help to identify what matters to you. By talking together you can develop a personal plan which can be shared with other services like Out of Hours Care and NHS24. This will help you get the right care if you become ill.”
This video explains why this is important.
