As we mentioned in a previous communication we are updating our online prescription request service so that it links directly with your patient record. This should make the whole process more efficient and reduce errors but we appreciate that changing the way this is done is a bit of a pain.
Whilst we make this transition the option to request prescriptions in the usual way will remain functional but you will not be required to log-in and the system will not store or have a record of your information.
The new system requires all users to complete a registration form
Please download and complete a form in either .pdf or Word .doc format:
Patient Registration Form – Online Services (Word doc)
Once this has been completed and returned to the practice we will issue you with a login and password that will allow you to access your prescriptions online and select any of your repeat items. You will also be able to request acute prescriptions through this option.
Completed forms should be returned to [email protected]
Please be aware that over 1200 patients currently use this service and there will be days when we get more forms than we can process. We will work through these as quickly as possible and update you via email when this has been done.
