When you attend the Practice to register you will be asked to complete a GP registration form, and new patient questionnaire form, these must be completed before you can see a Doctor.<\/p>\n\n\n\n
Acceptance on to the list is as at the discretion of the Doctor and we may ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).<\/em> Please note that your medical records may take several weeks to arrive at the Practice.<\/p>\n\n\n\n The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away.
